Med of the Week: Tamsulosin

Tamsulosin feature enlarged prostate and full bladder

In celebration of Men’s Health Week, we are highlighting Tamsulosin, a commonly prescribed medication for benign prostatic hyperplasia. While Tamsulosin can effectively alleviate urinary symptoms in men, it may also cause oral side effects. Keep the mind flowing by reading on:

Generic Name: Tamsulosin

Brand Name(s): Flomax

Pronunciation: tam soo LOE sin

About this drug:

  • Tamsulosin is used to improve urination in men who have benign prostatic hyperplasia (BPH).

Dentistry-related tidbits:

  • This medication may cause pain in the jaw or teeth.

  • Tamsulosin may cause xerostomia. Consider prescribing supplemental fluoride if the patient has a high rate of decay and increased frequency of dental exams.

  • Patients should limit drinking Alcohol as this combination may increase the risk of severe hypotension, orthostasis, and syncope.

  • Remind patients to be careful if they drive or engage in other activities that require them to be alert until they know how this medication affects them as Tamsulosin may cause dizziness and drowsiness.

  • Exercise caution when prescribing narcotics in combination with Tamsulosin to avoid an increased risk of CNS and respiratory depression, urinary retention, and diminished psychomotor function. For post-procedural pain, first attempt pain control by prescribing 400-600 mg of Ibuprofen (only if the patient does not have a history of hypertension) and 500-1000 mg of Acetaminophen at the same time. Take every 6 hours as needed for pain. Never exceed 4000 mg per day to avoid liver complications. Consider using Bupivacaine (Marcaine) local anesthetic to minimize post-op pain.

  • Monitor blood pressure when administering local anesthetics (e.g. Bupivacaine), narcotics, benzodiazepines, or the muscle relaxant Tizanidine, as combining these with Tamsulosin may increase the risk of severe hypotension, including syncope and orthostasis. Before prescribing any medication, use the Rx Check tool to prevent potential drug interactions or adverse effects.

Fun facts:

  • Tamsulosin is an alpha1 blocker that selectively targets alpha1 receptors in the prostate, resulting in smooth muscle relaxation and allowing urinary outflow.

  • Grapefruit products should be limited as this combination may increase levels and side effects of Tamsulosin.

  • A rare side effect that contributes to infertility is retrograde ejaculation. During ejaculation, most of the semen travels to the bladder due to the relaxation of the bladder sphincter.

  • In 2021, Tamsulosin was the 24th most commonly prescribed medication with over 23 million prescriptions.
